CTH Licensing Process Step by Step

The initial application process consists of the 5 steps outlined below. The whole process can take anywhere from 2 to 4 months.

1. Phone Screen
The licensing process begins with few preliminary questions we ask during our first phone conversation to see if you meet the minimum requirements. We then schedule an office meeting with you.

2. Office Meeting
You will be asked to fill out an
application form and bring it for our
office meeting. It is available on our
website or we can mail it to you if you
cannot access the Internet. The
application form inquires as to the
number of people living in the home,
their ages and relationships, the
address, a general description of the
home, and where the applicant is
employed. Please fill out the
application completely.

You will be meeting with Mary Pat DeCarlo, the Program Director, and Agnes Woroniecka, the CTH Coordinator. The interview will be approximately an hour long. It is a very important step during the process of licensing as it helps us to determine if you are the right candidate for the CTH. After the interview we will ask you to sign a release form for background checks (on all adults residing in your home). We will be also checking the Department of Children and Families for your record and check your references. Following our meeting we will schedule the Home Inspection within two weeks.

3. Home Inspection
During the Home Inspection your home is looked at by the CTH Coordinator to make sure it complies with the Department of Developmental Services (DDS) regulations. Any adjustments or alterations that need to be made to make it safe for the individual moving in will be reviewed. The home will go through a fire safety check. A local Fire Marshal’s certificate will be needed for those who live in anything other than a single family home; other reports may be needed based on the water and sewer source of the home. If there are any problems or deficiencies found in the home survey, the CTH Coordinator will help you to develop and implement a written plan of how to correct them. The major purpose of the Home Inspection is to prepare you for the DDS Licensing Inspection (5).
Procurement and the cost for any reports or necessary modifications are the sole responsibility of the applicant.

4. Training
After the Home Inspection, you will be scheduled for training and DDS Licensing Inspection. The training will include:

5. DDS Licensing Inspection
The licensing division of the DDS Central Office will conduct a survey of the home and determine compliance with the initial licensing standards. If the DDS Licensing Inspection is successful (everything is in place) your license will be issued.
You have to renew your license annually (see the Annual Renewal of the License). Also, during your first year as a provider, you will have a DDS Home Inspection additionally within 4 to 6 months of the initial licensure.
